MEET FLEETWOOD'S SOCIAL PRESCRIBERS


The Fleetwood Primary Care Network* is supported by a small team connecting residents with local groups and activities to support their wellbeing.


Wendy & Lisa have been keeping in touch with many residents by phone and more recently in person, socially distancing of course. They have also started to ask how individuals would like to be contacted and reconnected to the community in the future.


Prior to the lockdown, Fleetwood had a fantastic, vibrant network of things to do including men's and women's groups, singing, sports, arts and crafts, gardening and much more at venues like the Library, Lofthouse Building and Milton Street. It's great to hear that many of these groups have been carrying on using video technology and now looking at how they can restart safely in the weeks ahead.

The social prescribers will work alongside Healthier Fleetwood and all the partner organisations in the Town to develop the network of support and signpost residents to the groups and activities best suited to them.

In the meantime, taking advantage of this glorious weather, Wendy & Lisa have been out and about leaving some messages of positivity and kindness for residents and visitors to find, perhaps you'll spot one!


* A Primary Care Network is staff working together as a complete care community – in our case drawn from the Fleetwood GP surgeries, pharmacists, community, mental health and acute trusts, social care and the voluntary sector –  focusing on local population needs to provide care closer to patients’ homes. Fleetwood's Primary Care Network is recognised as an example of good practice for the level of involvement with residents through the Healthier Fleetwood initiative.
